If you purchase 2 Chi-Chis products, you can get a free reusable shopping bag. Just complete this form and mail it in with your original receipt(s) with the products circled. You will also need to include the UPCs from the 2 products that were purchased (liquor items are excluded).

Mail it to the address on the form and you will get your bag within 6-8 weeks. It does not specify that this offer is limited to one per household. However, it is only while supplies last and based upon purchases made from August 1, 2009 thru April 30, 2010. The completed form must be received by May 15, 2010.
